Starstruck

Registration Fee: $35 per year ($28 additional student same family) This annual fee is paid when you register for classes. You are not charged the registration fee for camps or special workshops. Registration fee will not be charged if you are a new student and register between March, April, or May. *

Tuition is due the 1st of each month, a $25 late fee will be added to your account if payment is not received by the 10th; we do not mail invoices or statements for tuition. A dancer is enrolled in class(es) until Starstruck is notified of a schedule change. Returned check fee is $30, we accept all major credit cards online and only Visa, Mastercard, and Discover at the office. If tuition is not paid by the 25th of the month the dancer will not be allowed back in class, they will be dropped from class, and if there is a waitlist their spot will be given to the next dancer on the waitlist. *

If, at any time, the client/or the party responsible for making payments under ths contract defaults by failing to timely make any payment when due, Starstruck may hire counsel, employ a collection agency, file a suit in law or equity, and may immediately cease further performance under this contract, all at Starstruck's sole election. Interest shall accrue on the amount of such debt at a rate of one and one half percent (1 and 1/2%) per month. Starstruck shall be entitled to an award of all attorney's fees and costs incurred in attempting to collect such debt. *

Photo and Video Disclaimer: I hereby permit Starstruck Academy of Dance to use photos or video taken in class or at performances in which my child may appear. Photos and/or video may be used for promotional or other materials of Starstruck Academy of Dance without payment or other consideration made to me. Dancer's name will not be posted with photos or video. *

Tuition is the same whether there are 3, 4, or 5 weeks in a month. The longer months make up for the shorter months. We are closed Memorial Day, Labor Day, Halloween, Wed through Sun for Thanksgiving, 2 weeks for Holiday Break in Dec and 1 week for Spring Break. *

Tuition, Camp, Workshop, & Recital fees are non-refundable and not transferable *

Credit is not given for missed classes. Students can arrange a make-up class in the same month of the missed class with the front desk. Students must be enrolled in the missed class to use the make-ups. Once a student drops a class the absences for that class are not available for make-ups. *

Add/Drop Forms: To add or drop classes please notify the front desk and fill out an add/drop form. *

Dancers must be picked up at the time their class is finished. Students must wait inside the building to be picked up. *

Accounts must be current with no outstanding balance for tuition, costumes, or other fees to participate in recital. *

You are financially responsible for tuition until Starstruck is notified in writing that the dancer is dropping class(es). We do not prorate tuition; the only exception is if you start in the middle of the month. *

COVID 19 Release An inherent risk of exposure to COVID 19 exists in any public place where people are present. We have taken enhanced health and safety measures. You agree to follow all posted instructions / guidelines and talk to your child about guidelines. By registering your child (children) for classes, camps, and events you voluntarily assume all risks related to exposure of COVID 19. *

I/we will not hold Starstruck Academy of Dance liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. *

I understand that Starstruck Academy of Dance is licensed and insured. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to a staff member as soon as possible. *

Liability Release for all camps, classes, virtual (online) classes, events, competitions, etc. As with other sports, there are risks involved with taking dance classes. Parents and students should be aware that injuries (including paralysis or even death) may occur in dance class. By enrolling your child in class at Starstruck (both in studio and virtually), you are assuming the risks involved with taking dance class. You are fully aware of the risk associated with your child participating in classes, virtual classes, camps and events, etc at Starstruck. You will talk with your child about the risk and demand they listen to instruction from their teachers. You acknowledge that you currently have and will continue to provide adequate insurance coverage for your child's protection. You agree to assume all risk, known or unknown, for participating in Starstruck's classes/camps in person and virtually. You realize the risk of injury involved and hereby agree to assume the responsibility of such for said student and further agree to save and hold harmless Starstruck, its employees, teachers, and all others concerned, and to indemnify them against loss. You jointly and severally hereby forever release, discharge, and acquit STARSTRUCK ACADEMY OF DANCE INC from any and all contracts, claims, suits, actions, or liabilities both in law and in equity specifically arising from, relating to or otherwise described as and limited to participation in any dance class including damages or injuries arising from or resulting from participation. This release shall be binding upon and inure to the benefit of the parties, their successors, assigns, and personal representatives. In case of emergency, you give your permission for emergency medical treatment. *
